The Disability Assessment Form represents the detailed medical history and confirmation of disability of the student to request for reasonable accommodation from the University. The form is duly filled by student & their medical practitioner; once the student has secured the admission into full and/or part time qualification at Iqra University. The assessment form is integral for the standing Accessibility Committee of Iqra University and the management to provide appropriate and reasonable support and accommodation to the students with disabilities to encourage them to study in a free & safe environment.

Following document is important in confirming the disability of the student and determining the specific need and support they require on campus. Hence the provided information should be completed by the registered healthcare professionals.

Identification for all types of disability is required to be disclosed by students so that we can confirm that you have a disability and to understand how your disability impacts your studies and campus life. In terms of a mental health condition, you are not required to disclose a specific mental health diagnosis, but confirmation of the presence of a disability which includes; the disability type (i.e. mental health), and the specific physical limitations must be provided.

Please note, upon disclosure of your disability condition, there will not be any adverse impact on your admission. Your admission will be processed in the same manner as other applications, without any discrimination on the basis of the disclosed disability.

Furthermore, a student’s personal health information collected from this form will be kept confidential within the standing Accessibility Committee, and is only disclosed on a need-to-know basis or otherwise in accordance with applicable legislation.
